I HAVE ATTENDED A SEALER TRAINING SESSION IN HUMANE HARVESTING. I UNDERSTAND THAT AS A HOLDER OF A TEMPORARY ASSISTANT SEAL LICENSE I AM NOT PERMITTED TO SHOOT A SEAL OR STRIKE A SEAL WITH A CLUB OR HAKAPIK UNLESS THAT SEAL HAS ALREADY BEEN SHOT OR STRUCK BY A LICENSED SEALER.

I understand that I may only participate in the 2012 seal fishery under the supervision of the sponsor, or designated operator, listed below.

I do solemnly declare that the information given on this application is true and correct and I make this solemn declaration conscientiously believing it to be true and knowing that it is of the same force and effect as if made under oath and by virtue of the Canada Evidence Act.

I understand that it is an offence under Section 61 of the Fisheries Act to knowingly make a false or misleading statement.
